Diabetes Education

Living with diabetes puts you at risk for nerve damage, high blood pressure and heart disease. But you can lower these risks by keeping blood sugar levels within a healthy range. Having a balanced diet and becoming more physically active also helps.

These efforts involve changes that don’t always come easy, which is why Trinity Health Michigan offers diabetes education services. It’s also known as diabetes self-management education because it empowers you to take control of your health.

Our experienced team gives you the information and support you need to live a full and active life. Whether you have prediabetes, Type 1 or Type 2 diabetes, we can help you feel your best.

Certified diabetes educators use research-based methods to provide comprehensive diabetes education. Our dedication to delivering effective diabetes self-management education has helped Trinity Health Michigan earn national recognition.

We maintain accreditation from the:

  • Association of Diabetes Care and Education Specialists (ADCES)
  • Michigan Department of Health & Human Services (MDHHS)

Our Locations

Is Diabetes Self-Management Education Right for Me?

You may be eligible for diabetes education if you:

  • Are newly diagnosed
  • Have Type 1 diabetes, Type 2 diabetes or prediabetes and have never completed comprehensive diabetes self-management education
  • Had changes in your treatment plan

A physician referral is required. Medicare, Medicaid and most private insurance plans typically cover the cost of diabetes education. If you’re on Medicare and have previously attended diabetes education classes, you may be eligible for an annual two-hour review session with one of our nurses or dietitians.

Diabetes Self-Management Education: What to Expect

Diabetes education involves individual and group learning sessions. We offer morning, afternoon and evening options. Your loved ones are welcome to attend to help you maintain healthy diabetes self-care practices.

Participating in diabetes education teaches you to:

  • Understand diabetes and how it affects your body
  • Establish care goals and make steady progress toward achieving them
  • See how nutrition, exercise and medication help control blood sugar
  • Prevent diabetes complications
  • Plan for pregnancy or manage diabetes care during pregnancy

Additional education topics help you incorporate diabetes care tasks into your daily routine. These may include:

  • Checking your blood sugar
  • Planning and eating nutritious meals
  • Controlling your weight
  • Using insulin
  • Administering insulin pump therapy if necessary

One-on-One Diabetes Self-Management Education

Our services also include one-on-one sessions with certified diabetes educators and registered dietitians. We tailor education to your needs and provide personalized recommendations for meeting your goals.

Education topics may include:

  • Continuous glucose monitoring and insulin pump training from our certified pump trainer
  • Gestational diabetes care for women who experience diabetes during pregnancy
  • Insulin or incretin administration
  • Self-blood glucose monitoring
