Insurance Information:
Diabetes Education is reimbursed by Medicare, Medicaid and most private insurance plans. If you have previously attended Diabetes Education classes, Medicare covers a two-hour annual education review with either or both the nurse and dietitian.

The Diabetes Program facility is connected to the Chelsea Wellness Center. Park in the lot in front of the building.
