Clinical Environment/Program Focus
Community Hospice: 2 rotations at various locations
Location: Faith Hospice
Supervising Attending: Patrick Fitzgibbon, DO
Location: Spectrum Health Hospice
Supervising Attending: Simin Beg, MD
Description: This rotation consists of two (four week) rotations involving exposure to outpatient hospice care. This rotation will provide exposure to the concepts of adult hospice care in the home, and non-hospice facility setting. The hospice physician’s essential role with concern to home hospice is to act as medical consultant to the attending physician, the hospice nurse, and the entire hospice care team to ensure the coherence of the medical aspects of plan of care for the hospice population served.
The fellow may act as attending for hospice patients who do not have another attending physician. While on the hospice rotation, the fellow will also develop a good understanding of the hospice benefit and the regulatory environment to ensure his ability to serve as medical director to a hospice agency.
Inpatient Hospice Unit: 2 rotations
Location: Faith Hospice/Trillium Woods
Supervising Attending: John Mulder, MD
Description: This rotation consists of two (four week) rotations involving exposure to inpatient hospice care. This rotation will provide exposure to the concepts of adult hospice care in a hospice facility. The hospice physician’s essential role with concern to inpatient hospice is to plan and coordinate care with the hospice nurse and the hospice care team to ensure the coherence of the medical aspects of plan of care for the hospice population served.
The fellow may act as attending for hospice patients who do not have another attending physician. While on the hospice rotation, the fellow will also develop a good understanding of the hospice benefit and the regulatory environment to ensure his ability to serve as medical director to a hospice agency.
Inpatient Palliative Service: 6 Rotations Total
Location 1: Trinity Health Grand Rapids (3 rotations)
Trinity Health Grand Rapids Supervising Attending: Joel Phillips, DO and Brett LeVesseur, MD
Description: This rotation consists of three (four week) rotations involving exposure to inpatient and outpatient palliative medicine. This rotation will provide exposure to the concepts of palliative medicine in the hospital and clinic setting. The HPM physician’s essential role is to serve as a tertiary-level specialty consultant to referring physicians (primary physicians, hospitalists, other specialists).
Location 2: Corewell Health (3 rotations)
Corewell Health Supervising Physician: Gretchen Roe, MD
Description: This rotation consists of three (four week) rotations involving exposure to inpatient palliative medicine. This rotation will provide exposure to the concepts of palliative medicine in the hospital setting. The HPM physician’s essential role is to serve as a tertiary-level specialty consultant to referring physicians (primary physicians, hospitalists, other specialists).
Long Term Care: 1 rotation
Location: Holland Home www.hollandhome.org
Supervising Attending: Rob Riekse, MD
Description: This is one (four week) rotation. The fellow will gain the knowledge and skills necessary to (1) define with the patient the goals of care, (2) formulate plans of care of acute and sub-acute medical processes, (3) routinely assess geriatric patients for the common geriatric problems listed regardless of the admitting diagnosis, (4) evaluate and facilitate the coordination of care (medical, nursing, rehabilitative and functional support) required by for the patient experiencing complex medical and social issues.
