Yes. In the State of Michigan, unless you do not have insurance, you need a prescription from your doctor to be able to receive physical or occupational therapy. You should let your doctor know that you would like to come to Probility Physical Therapy for your treatment. Only those without insurance may come to therapy without a script on a self-pay basis.
You should bring a prescription from your referring physician, your driver’s license and insurance card. We also recommend that you fill out the new patient forms (PDF, 959 KB) and bring them with you to your first visit. If you do not do this, we ask that you arrive at least 20-30 minutes before your initial appointment to allow ample time for filling out the paperwork.
It is beneficial if you have shoulder or back problem to wear or bring a tank top. It is also helpful to wear or bring shorts if we are evaluating knee and foot/ankle problems.
Your initial evaluation and all follow up sessions should be expected to last between 90-120 minutes. At your first appointment, your therapist will perform an initial evaluation, perform manual therapy techniques to treat the problems that we find and create a specialized rehabilitation exercise program for you.
We understand that situations arise in which you cannot always make your scheduled appointment. If that is ever the case, we ask that you call the clinic where your appointment is scheduled at least 24- hours before your scheduled appointment.
All canceled appointments must be rescheduled within the same week.
