Livonia Community Support


Trinity Health Livonia is pleased to announce the Community Impact Grant Program. This program is designed to support organizations working in Western Wayne County to address priority health needs as identified by the Community Health Needs Assessment (CHNA) and resulting Implementation Strategy. FY25-27 priority needs are:

  • Mental health and substance use
  • Diabetes/obesity/high blood pressure
  • Access to resources, with a focus on housing and nutrition security

Award Amount:

  • Successful proposals will be funded with a one-year grant, up to $25,000, with the opportunity to apply for continuation funding in subsequent years.

Eligibility Requirements:

  • The use of funds must align with one of the Trinity Health Livonia Community Health Needs Assessment priority needs.
  • The program must address a CHNA Implementation Strategy or identify a new tactic that would complement the plan.
  • Eligible organizations include community-based organizations, and local governmental entities (i.e. a school district, a public health department, a housing agency/authority)
  • Community-based organizations must operate within or primarily serve residents of Western Wayne County.
  • Priority will be given to projects focused on 48185, 48186, 48239, 48240, 48141, 48127 (Dearborn Heights) 48228 and 48219.

Community Impact Grant Scoring Matrix:

Proposals are scored based on fit with three scoring categories – (1) Funding Intent, (2) Project Feasibility, and (3) Financial Feasibility. Funding Intent, the most valuable category at 50% of the available points, describes whether the proposed project fits within the mission and intent of the funding opportunity in general. Project Feasibility measures whether a particular project is properly designed and is likely to be successful operationally. Financial Feasibility measures the completeness and strength of the budget presented for the request. Each section is broken into subcategories that will receive individual scores by the reviewers.

Reporting Requirements:

  • Submission of a mid-year progress report (template will be provided)
  • Submission of a final report
    • A description of how the proposal was implemented.
    • A review of how the funds were spent, including a detailed final budget showing all expenses.
    • A statement indicating your plans for the program, including sustainability.

FY26 Program Timeline:

Release the Request for Proposal June 1, 2025
Applications Due June 30, 2025
Award Announcement August 15, 2025
Project Period October 1, 2025 - September 30, 2026
