Investing in Our Communities Program
Trinity Health Ann Arbor and Livingston are pleased to announce the availability of funds for the Investing in Our Communities Grant Programs.
Award Amount:
- Successful proposals will be funded with a two-year grant up to $50,000/year
Eligibility Requirements:
- Funds must be dedicated toward community benefit purposes. Community benefits are programs and activities that provide treatment or promote health in response to an identified community need and meet at least one of the following criteria: Improve access to health care services, enhance health of the community, or advance medical or health knowledge.
- Use of funds must align with one of the Trinity Health Ann Arbor and Livingston FY2025-FY2027 Community Health Needs Assessment priority needs (listed above) and organizational mission.
- Eligible organizations include community-based organizations, faith-based organizations, and local governmental entities (i.e. a school district, a public health department, a housing agency/authority)
- Agencies must operate within or primarily serve the following service area: 48197, 48198, and Livingston County. Programs that serve populations outside of these regions will receive much lower priority for funding.
- First year funding must be spent prior to obtaining year two funds.
- Funds may be used for programmatic purposes (i.e. personnel, supplies, training costs)
Applications that contain the following elements will receive priority consideration:
- Ability to measure and report on outcomes-based success metrics
- Preference will be given to proposals that leverage additional financial/in-kind support and include collaborative partners on the application
- Promote innovative and scalable solutions
Ineligibility Criteria:
- For-profit entities, including start-up small businesses
- Grants specifically for travel
- Funding to support or purchase Trinity Health services
- Organizations currently receiving more than $100,000 from Trinity Health are ineligible for CHNA program. However, these organizations are still eligible for the CER Program.
- Current FY25-26 recipients of Trinity Health Investing in Our Communities two-year grant
Investing in Our Communities Decision Matrix:
The Trinity Health Ann Arbor and Livingston Community Health Council (including board members and colleagues) will review and score the applications. A value point system will be used in the determination of the Investing in Our Communities grant program. The system is established with a 1-5 point scale, with (1) providing no apparent alignment/value/impact and (5) providing the greatest alignment/value/impact.
- Alignment with Trinity Health Ann Arbor and Livingston Community Health Needs Assessment & Community Health & Well-Being Strategy
- Alignment to institutional mission
- Alignment with priority service areas
- Impact on Health Equity
- Reach of initiative (Number of community members positively impacted)
- Sustainability plan/leveraging other resources
Reporting Requirements:
- Presentation to Trinity Health Leadership, upon request
- Submission of a mid-year progress report (template will be provided)
- Submission of a final report (1000 words or less) and final presentation.
- A description of how the proposal was implemented.
- A review of how the funds were spent, including a detailed final budget showing all expenses.
- An evaluation of the success of the program. Were the objectives met?
- A statement indicating your future plans for the program, including sustainability.
- Please list out the ways in which your organization recognized Trinity Health Ann Arbor/Livingston
Award Amount:
- Successful proposals will be provided with an AED (if desired), replacement batteries and pads, training and certification on hands-only CPR, and all appropriate training needed to implement their own Cardiac Emergency Response Program (up to $5,000).
Eligibility Requirements:
- Eligible organizations include community-based organizations, faith-based organizations, and local governmental entities (i.e. a school district, a public health department, a housing agency/authority)
- Agencies must operate within or primarily serve the following service area: 48197, 48198, and Livingston County. Programs that serve populations outside of these regions will receive much lower priority for funding.
Applications that contain the following elements will receive priority consideration:
- Ability to demonstrate a dedicated individual or group that will provide the necessary infrastructure to support implementation of the CER Program
Ineligibility Criteria:
- For-profit entities, including start-up small businesses.
- Organizations who provide cardiac emergency response training and trainer AEDs
