Neuro-palliative Care
Our neuro-palliative care physicians optimize quality of life for people with neurological conditions that have long-lasting or life-altering circumstances. We are available at any stage of illness and typically get involved early in a person’s care.
Neuro-palliative care physicians work with patients, families and other neurological disease specialists to ease the burden of living with certain chronic disorders or a life-altering condition. Services may include pain management, care coordination and more. We also guide patients and families through conversations about future care needs and how to communicate care preferences.
Neurorehabilitation
Neurorehabilitation involves a variety of therapies that optimize your safety, comfort and independence. Physical medicine doctors develop a personalized care plan and work with highly skilled therapists to coordinate your care.
Neurorehabilitation may include:
- Occupational therapy, which teaches you methods for coping with changes to your abilities. Therapists may teach you how to use devices that help you walk, eat and perform daily tasks.
- Physical therapy, which may involve balance training to help you feel steady on your feet. Therapists may also guide you through gentle exercises to improve strength and endurance.
- Speech therapy to help you communicate. We teach you new techniques for using your voice and expressing yourself when words do not come easily.
- Swallow evaluations, to check how well you can swallow safely. Our experts use special tests to see how you swallow, making sure you're safe and comfortable when you eat and drink.
Nurse Navigators and Care Coordinators
Nurse navigators help patients and families stay on top of care plans for complex conditions. Navigators are nurses who specialize in caring for patients with neurological conditions. We help you make appointments, answer questions and provide encouragement when you need it. Our goal is to ensure you have an excellent experience with our programs.
Care coordinators guide your transition home or to a rehabilitation facility after a hospitalization. These efforts ensure you continue receiving the therapies, medications and follow-up care you need to recover. Our family-centered approach includes helping the entire family prepare and learn what to expect.
Social Work
Our social workers specialize in helping patients with neurological disorders and their families.
They help people navigate challenges by providing:
- Counseling for depression, anxiety and relationship issues
- Referrals to helpful resources, such as local transportation and wellness services
- Guidance about disability benefits and other government programs patients may be eligible for
- Support as families cope with changes to their loved one’s health, independence and living arrangements
Spiritual Care
When you are a patient or supporting a loved one with a neurological condition, healing comes from many sources, including your faith. Our extended team includes chaplains who promote holistic healing through spiritual care. They provide comfort, hope and prayer that give many people peace of mind.
Trinity Health Michigan honors the traditions and customs of people from all religious and spiritual backgrounds. Patients and visitors are welcome to visit our chapels for private prayer. We also hold mass a few times a week.
Services and Support
Support groups help patients and families feel less alone in their neurological disease care journey. Members of our team lead conversations that help participants get to know one another and learn from each other. Trinity Health hosts a variety of support groups, many of which you can participate in virtually.
