Our Care Model
We work in tandem with your current providers to create a roadmap for your care. Beginning conversations early in your disease management can better prepare you and your loved ones for the road to come.
Our program incorporates the following:
- Physical, psychological and cognitive problems including memory, grief and anxiety
- Counseling for spiritual, social and psychological concerns related to their disease
- Future planning and transitions in care: end of life choices, advanced directives and hospice care
- Access to community resources
- Communication between caregivers and patients
- Caregiver support and respite
