Welcome to Trinity Health IHA Medical Group, Primary Care - Domino's Farms

We are a Trinity Health IHA Medical Group practice that provides care for adolescents aged 16 to adults. Our physicians and staff
provide the highest quality medical care to our patients and the best service to their families. We have always appreciated the opportunity to care for our existing patients and welcome new patients to join our care.

Trinity Health IHA Medical Group Primary Care - Domino's Farms offers same-day appointments for existing patients. If you have an urgent medical problem and need assistance after business hours, please call the office and your call will be directed to the Trinity Health IHA Call Center. The call center staff will assist you and contact the physician on-call to address your concerns.

Trinity Health IHA Medical Group Primary Care - Domino's Farms is a designated Patient-Centered Medical Home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support needed to make decisions and participate in their own health care.

As a practice, we know that preventive care, including vaccinations, is the most important part of our service to patients and their
families. To best protect your children, our other patients, as well as our staff and providers from vaccine-preventable illness, we have made the decision to only accept families into our practice who plan to fully vaccinate their children according to the CDC, American Academy of Pediatrics and American Academy of Family Physicians guidelines.
