Asthma Network
Trinity Health Asthma Network provides home-based asthma education and case management. The goal of the Asthma Network is to improve the health and quality of life for individuals with asthma by providing support, education, and referrals to community resources. These tools are helpful to the patient and family.
Asthma is a chronic disease of the lungs. A chronic disease is a condition that is persistent or otherwise long lasting in its effects or a disease that comes with time. People who have asthma have inflamed, or swollen/narrow airways, which can cause them to have a hard time breathing. Less air can get in and out of the lungs – it is especially hard to get all the air out of the lungs.
Trinity Health Asthma Network provides six to 10 in home visits depending on insurance authorization and patient's specific needs. Call your medical provider for a referral.
