To promote communication and relationship-building before, during and after transplant – with both patients and referring providers – the role of outreach liaison was established. Jill joined our team in August 2022 and brings with her 15 years of experience with kidney transplant and a passion for providing patient and staff education.
She is available to assist patients, providers and staff with general transplant questions, update requests or specific patient-related information.
Contact our outreach liaison to:
- Get answers to questions regarding referral to transplant or evaluation testing requirements
- Arrange for regularly emailed updates on referred patients
- Obtain specific patient status updates
- Arrange for staff education sessions
- Set up on-on-one patient education sessions
- Schedule nephrology office or dialysis unit visits and/or lobby days
