To strengthen communication and collaboration between our transplant program, patients and referring providers, the Outreach Liaison role was established. Jill Wagner joined the Kidney Transplant Center in 2022 and brings more than 15 years of kidney transplant experience, along with a passion for education, relationship-building and patient advocacy.
Jill serves as a dedicated resource for patients, nephrology practices, dialysis centers and healthcare professionals. She is available to answer general transplant questions, provide patient status updates, facilitate communication with the transplant team and support educational initiatives throughout West Michigan and beyond.
Contact our Outreach Liaison to:
- Get answers to questions about kidney transplant referral criteria, evaluation requirements or the transplant process
- Receive updates on referred patients and transplant candidacy status
- Request patient-specific status updates
- Coordinate educational presentations for nephrology offices, dialysis centers and community groups
- Schedule individual or group patient or staff education sessions
- Arrange nephrology office visits, dialysis center visits and outreach events
- Explore partnership opportunities to improve transplant access and patient outcome
