Billing
Trinity Health Probility Physical Therapy will be the main contact for all Patient Billing Services. Your Probility bill will come from Trinity Health and will also include all other Trinity Health providers and facilities, including all of our Trinity Health Michigan hospital sites, for your convenience. It will include an account summary, as well as detailed information about each visit.

Online
Online payment can be submitted through the MyChart Patient Portal. You do not need a MyChart account to pay your bill. If you have a MyChart account, you can also pay your bill through the Trinity Health MyChart mobile app. See the instruction links below.

By Phone
To pay by credit or debit card over the phone, please call 800-494-5797 between 8:00 am and 5:30 pm EST.
By Mail
Fill out the bottom portion of your bill and return it with your payment to the address on your bill.
Financial Assistance
