Patient and Family Advisory Council
Trinity Health Oakland is committed to delivering patient-centered care. Former patients, family members and the community are invited to serve as volunteer Patient and Family Experience Advisors to offer input and advice on various aspects of the patient experience.
Advisors serve in many ways – on councils, committees, electronic advisors and one-time focus groups.

Your participation in helping us improve patient care policies, processes, communication materials, the physical environment, quality improvement activities, and safety efforts, will have a direct impact on our patients' experiences while receiving care in the hospital, or at any of our facilities.
Available Opportunities
Based on your area of interest and/or expertise, we offer the following opportunities:
Council Member (minimum 1-year commitment required)
Membership on a hospital committee or team with regular meetings, such as:
- Patient Experience
- Pain Management
- Quality and Safety
- Complaint/Grievance
Attending additional meetings and activities, for example:
- Assisting in the design or improvement of a new or existing program, service, policy or process.

Focus Group Participant
Offering opinions and perspectives based upon your experience with a particular aspect of care, a department, or type of illness. Meets 1-2 times to cover a specific topic.

Electronic Advisor
Periodic requests for:
Feedback on brochures, patient education materials, websites, policies, newsletters and other health care consumer materials. Participation can be completed electronically and remotely from the comfort of your own home. This only requires being able to access documents via email for your review and comment.

Experience Sharing
Share personal health care experiences with departments and employees throughout the health system. Your experience may have been a very positive experience where we can help to reinforce the type and manner of care you received. It may also be an experience where changes need to be made in order to improve care. The key is being able to share your story in a way to inspire and motivate.

All applicable volunteer requirements will apply, such as formal application, health screening, background check, references and confidentiality forms signed, and regular documentation of time spent volunteering.

Key Requirements
There are certain key requirements that we ask each volunteer to meet in order to volunteer at Trinity Health. These requirements are set in place for your protection as well as for the protection of our patients, visitors, and staff. Each requirement must be met before your first day of volunteering. The list below contains some of our requirements; however others may also apply:
There are certain key requirements that we ask each volunteer to meet in order to volunteer at Trinity Health. These requirements are set in place for your protection as well as for the protection of our patients, visitors, and staff. Each requirement must be met before your first day of volunteering. - Application - Complete and submit the Download Application (PDF 326KB)
- Interview – All advisors will meet with Patient and Community Engagement staff to discuss interests, complete paperwork and determine potential placement.
- References – All advisors will need to have two reference forms completed by non-relatives Click link to access reference forms
- Background Check – Criminal background checks are conducted on all volunteer advisors.
- Health Screening – Vaccine requirements include: Tdap Tetanus, Diphtheria, Acellular Pertussis – Vaccination required in past 10 years.
- TB Screening – TB skin test required in past 12 months – Can be done here (free of charge). If you have had a previous positive TB test, please provide a copy of the chest x-ray.
- Influenza Vaccine – It is a volunteer requirement to show proof of receiving this season’s flu vaccine or a dose will be administered at no charge to you.
- If no documentation can be found, you may sign a declination form for the following: Varicella (Chickenpox) vaccine and Measles, Mumps, and Rubella (MMR)
- Training/Orientation – As an Experience Advisor you will participate in an orientation to the program.
- Name Badge – Each Experience Advisor will receive a hospital-issued identification badge to wear at all times they are functioning in this role.
