PACE

LifeCircles provides continuous, high-quality care for seniors, supporting their independence to live at home. Eligible seniors receive personalized care from a dedicated interdisciplinary team that coordinates services to meet each participant’s unique health needs. At our Center, participants benefit from clinical and rehabilitation therapy while building relationships with doctors, nurses, and peers—all in one convenient location.

As a Program of All-Inclusive Care for the Elderly (PACE), we offer a multi-faceted approach, with services provided at no cost for those eligible for Medicaid or both Medicaid and Medicare. For individuals not eligible for Medicaid, a monthly fee applies.
