If you have a loved one who is suffering from a complex, life limiting disease who has pain, nausea, or is frequently in the hospital, you may want to consider Palliative Care.
Palliative Care focuses on managing symptoms, controlling pain, improving quality of life and relieving stress for patient and family. The goal is comfort for someone who has a complex, chronic illness that cannot be cured, only managed. It does not replace your primary treatment.
Palliative Care provides:
- Improved quality of life for patients and their families
- Emotional support and education for you and your family
- Coordination of care with your primary care physician
- Guidance in understanding difficult and complex treatment options, including management of your symptoms
- Expert management of pain and other symptoms of illness so you get the best relief possible
- Home visits
What to Expect
A nurse case manager evaluates each patient, with our Palliative Care team and your physician, will guide you to achieve your goals of comfort and symptom management to maintain your highest quality of life.
The evaluation includes:
- a physical assessment and recommendations for pain and symptom management
- a review of your current medications
- support in developing goals for treatment and care.

Anyone with a serious chronic illness, such as Advanced CHF, COPD, Heart Disease, Kidney Disease and Dementia.
Palliative Care is not the same as Hospice. Hospice Care is meant for those in the final months of life. Unlike Hospice Care, you can receive Palliative Care as the same time as curative and aggressive treatment.
Most insurances plans, including Medicare and Medicaid cover Palliative Care services. There will be a co-pay for the physician consultation.
