Although we do not require a referral, obtaining one from your doctor may assist in insurance reimbursement.
The monthly culinary classes are free. For all other offerings, we encourage you to check with your insurance provider to see if they will cover the cost of the program. The Foundations of Lifestyle Medicine class series, personalized lifestyle medicine consultations and medical nutrition therapy appointments are billed as Medical Nutrition Therapy (CPT Codes: 97802, 97803, 97804). Some insurances may require a referral from your doctor in order for our services to be covered. For any costs that are not covered by your insurance, you will receive a bill. You can make a payment through the mail or your MyChart account using your debit or credit card. Flexible Spending or Health Savings Accounts may be used to pay for these services as well.
You can choose to schedule your individual consultation through a virtual meeting platform or in person. The free culinary sessions are fully virtual. Please ask about in-person and virtual options when scheduling your individual consultation or inquiring about group classes.
Yes! Lifestyle Medicine complements traditional care, it doesn’t replace it. Lifestyle Medicine visits will not replace any medical care you’re already receiving. Your Lifestyle Medicine visits will be documented in your electronic health record or MyChart account so your other health care providers will have access to the information.
Most of our programs and services are for adults 18 years or older. If you are a family with children up to the age of 18 looking to learn about healthy eating, an active lifestyle and effective communication to promote well-being, please connect with the Trinity Health Healthy Families Program.
