It drives many of us crazy that we have to fill out on-line questions every time we go for any kind of doctor's visit. Then when we meet the doctor/nurse/practiionser, they ask us the exact same questions over again (regarding mental health, lifestyle habits, etc). Your software needs better coordination with what the provider sees or else the provider needs to actually look at the answers we give before meeting with us. My doctor said he's used to asking questions--fine, but then don't make us fill out 10 pages of questions on-line ahead of time.

This was my first visit with this physician. I decided to change from my usual physician to a new physician due to her location and difficulty scheduling appointments when needed.

Dr. Bruderly does a very good job. For example, Dr. Bruderly examined and explained the probable cause of a bump in my lower lip. My later reading about this sort of thing has shown that Dr. Bruderly is most probably correct. (Contrast this with a previous doctor who didn't even look closely at the lip and who agreed with my erroneous assumptions about the bump.)
