Dr. Sleziak has always done a great job on my lower lumbar ablations. I appreciate his skill level to get me as close to "pain free" as is possible at this age. This is the first time he is managing me for everything vs. being referred by Lukasik. I hope I did a good enough job explaining what the process is for when my back flares and starts causing problems before the next ablation in November of 2024.

Dr. Sleziak is a fantastic provider. The procedures he performs on my back help to manage my pain thus allowing me to live a normal/active life. He always listens when I tell him it is time for another procedure. He is a great guy!!!
He showed me X9rays and did not bother with explanations of films form HFAH [ . . . ]. He did show me & explain the one from his previous procedure and not terribly informative.
I understand why, but wish Dr. S would type in note after we have a conversation, not while I'm talking.
