Following completion of a comprehensive Community Health Needs Assessment (CHNA), Trinity Health Grand Rapids is collaborating with community stakeholders to create an Implementation Strategy to address the top 3 priority needs identified by residents of Kent County: Housing, Health Insurance, and Access to Medical Care.

"This recent assessment is the voice of our community, it tells us what their priorities are, and confirms what our focus should be,” said Matt Biersack, MD, president of Trinity Health Grand Rapids. “Our team is eager to get to work addressing these needs, making Kent County a healthier and more equitable place to live, work, learn, and play.”

“Our implementation strategy will also seek to address health inequities that impact individuals from many community segments, including people of color, members of the LGBTQ+ community, immigrants, refugees, people with disabilities, and veterans,” said Jennifer Chaffer, Director of Community Health & Well-Being, Trinity Health Grand Rapids. “The strategy will be finalized and go before the hospital Board of Trustees for approval in September.”

One example in West Michigan is Trinity Health Grand Rapids’ embedding Community Health Workers in physician offices to assist individuals with applying for Medicaid, Trinity Health Financial Assistance, and other benefit programs.
