Trinity Health Ann Arbor First Hospital in Michigan to Earn Heart Failure Accreditation with Outpatient Services Designation

ANN ARBOR, Mich. (June 17, 2024) – The American College of Cardiology (ACC) has recognized Trinity Health Ann Arbor and its Trinity Health Michigan Heart — Ann Arbor physician practice for its demonstrated expertise and commitment in treating patients with heart failure. Trinity Health Ann Arbor is the first hospital in the state of Michigan to earn the ACC’S Heart Failure Accreditation with Outpatient Services.
Accreditation is provided to medical facilities based on rigorous onsite review of the staff’s ability to evaluate, diagnose and treat patients with heart failure through pre-hospital care, early stabilization, acute care, transitional care, clinical quality measures, and more.

Hospitals like Trinity Health Ann Arbor that earn this accreditation are proven to employ an evidence-based, protocol-driven and systematic approach to managing heart failure. These hospitals are also able to identify and predict high-risk patients, and have been able to reduce treatment times, the patient’s total time in the hospital, and readmissions. Additionally, they demonstrate the management of acute heart failure, as well as partner with outpatient clinics for heart failure patients to be provided with post-discharge care and services. This approach provides a smooth transition between the inpatient and outpatient setting, and ensures continued care post-discharge.
