Your Lifestyle Medicine visits will be documented in your electronic health record or MyChart account so your other health care providers will have access to the information.

Most of our programs and services are for adults 18 years or older. If you are looking for a program for a middle school aged child, consider Nutrition Buddies. If you are a family with children up to the age of 18, please connect with the Trinity Health Healthy Families Program.

If you were recently diagnosed with a new chronic condition such as diabetes or heart disease, we recommend pursuing disease-specific education prior to enrolling in a Lifestyle Medicine class series. For more information about Trinity Health Diabetes Education, click here. For more information about Cardiac Rehab and Intensive Cardiac Rehab, follow the links.

If you are struggling to control your blood sugar levels, we recommend pursuing support with our diabetes educators prior to enrolling in a Lifestyle Medicine class series. For more information about Trinity Health Diabetes Education, click here.

Optional one-on-one consultations are available with a lifestyle medicine board-certified Registered Dietitian, virtually or in-person, after completion of a Lifestyle Medicine class series. These appointments are billed to your insurance.
