Hospice

Trinity Health Home Care and Hospice provides 24/7/365 support, comfort and compassionate care for patients with serious, possibly terminal illnesses. Surrounded by the support of their loved ones, our patients live free of pain in the comfort of their own homes. Our care is given to respect the dignity and uniqueness of each individual.

In collaboration with the patient’s physician, we provide patients with pain management and symptom control. We also give psychological, emotional, social and spiritual support for patients and their families. Through our healing bereavement services program, we help family members and loved ones grieving the loss of a loved one.
