Giving Back

Probility's mission of making a difference in people’s lives extends well beyond our clinic doors. We believe in being an integral part of the communities we serve and contribute more than $50,000 annually to local charities and sponsorships.

Would you like Probility to sponsor your event or organization? Please review the following eligibility criteria and submit your email requests directly to probilityinfo@trinity-health.org.

Eligibility

To be eligible for contributions, organizations must:

• Qualify for nonprofit status under the Internal Revenue Code & provide proof of nonprofit status.
• Benefit the local community served by Probility Physical Therapy.
• Demonstrate sound and responsible financial policies and management.
• Provide a description of your organization and project, including purpose and objectives, on agency letterhead signed by the executive director.
• Compliance with the Equal Employment Opportunities Act and other federal, state and local laws and regulations.
• Provide Board of Directors list.
• Give brief history of your organization.

Contributions are made principally in the areas of education, the environment and human services; for example, assisting in health and housing and in combating hunger.
